In order for us to serve you better please plan to arrive 20-30 minutes early for your appointment for registration and check in. If you are unable to keep your appointment for any reason, please call us at least 24 hours prior to your scheduled visit so we may assign the time to another patient. BILLING INFORMATION:
When treatment is received in a UCLA Hospital Based Clinic, there are fees associated with the treatment room, which is identified as a Facility Fee and fees for the medical provider, the Physician.  The Facility Fees are billed by the hospital and covers the cost for the use of the space and/or equipment.  Please note that this is a billable and appropriate charge for the use of the facility in which the care was rendered.  It is accepted and reimbursed by most insurance health plans."

We strongly advise you to call your insurance company to determine whether they cover preventive OR diagnostic services. You will be responsible for charges not paid by your insurance carrier. Co-payments are collected at the time of your visit. If you do not have insurance please be prepared to pay at the time of your visit.

Please bring the following items with you to your visit :

• A copy of your insurance card (Medicare or Medi-Cal card if applicable) showing your policy number, group or certificate number and if possible, insurance billing form.
• New Patient Questionnaire (DO NOT MAIL)
• A list of your current medications, vitamins, and supplements

HOURS:
The Iris Cantor-UCLA Women’s Health Center is open Monday through Friday from 8:00 am to 5:00 pm and 9:30 am to 5:00 pm on Wednesdays. To avoid delays in seeing your physician, please try to keep all of your scheduled appointments. We will try to accommodate urgent appointments whenever possible.

PARKING
The parking fee is $8.00. We do not validate.
